Just wanted to say thanks to Leonid for his help to resolve this issue.
For the benefit of anyone following this discussion:
"On this page https://flast.com.au/studio/builder_menu.php?page=items&module=system&set=sys_toolbar_member you may see that menu items are available for the certain permissions levels. As you set the limits then new levels will not be there right after creation, It is required to add them manually"
