I'm beginner with UNA , i use people app but how i can get two different profiles for two different people with different access level such as doctor and patient in medial website ?!

Welcome to abord! In your case, you will need separate accounts for the doctors and patients. You may divide them by permissions levels https://github.com/unaio/una/wiki/Permissions-Builder
