A new form can be created via Developer app. You may find there the "Form" tab. But as the new form requires the new separate table anyway the best way is described here https://github.com/unaio/una/wiki/Dev-Forms with the some code work.

  I see this https://github.com/unaio/una/wiki/Dev-Forms is including relation to a module. I won't create a new module. Just want to create a form.

  No, this example (especially form this part) shows how you may "call" in PHP code (of any module or page) your new form.
