Thanks for getting back to me!
I've been using the manual so far but I still can't seem to get the form rendered after creating all the fields. More specifically is there a way to view the form from the pages builder, as I can't seem to find an option for it?

So it is better to have your own module which will call the service method with the code like from the given examples. The manual on how to create your module is located here:
