      Your screenshot shows order manual processing form while you are asking about manual withdraw. Could you please describe your question in more details.
      P.S. If possibility to withdraw is enabled in creadits. User may send a withdraw request to site admin specifying the amount of creadits he wants to withdraw. Admin should manually send requested money to the user and then confirm the withdraw via Dashboard -> Credits -> All History -> Confirm withdraw.
