I guess the list of mentions can be reduced to the first position only :-) But commonly, you need to find the Add form of this mod in the Studio->Forms or Studio->Developer->Forms areas and disable all less fields there.

  Not sure I understand your information but here is what I got when I check the form section. I noticed, there is no option to disable or edit them. Please How do I make this possible?
