  All of those requirements. If that app includes it and has it, why did my submission get rejected

  - Make sure you have the EULA/Terms pages filled with your specific terms - they' re blank by default.
  - Look at your content action menus (like actions in People profiles, in Posts, etc) and make sure that "Report" button is active and named correctly. Also make sure that in Permissions the report action is enabled for standard users.
  - In your Terms make sure to add an explanation that all reports are processed within 24 hours and all objectionable content will be permanently deleted or hidden and won't be available for browsing by normal users.
