The Profile Cards module operates with the same features which are available to a default profile card. So if you need some "label" (meta info) to be available then you have to add it to a profile card's meta info menu. But as far as I see you've already managed it.

Yes and no. Yes, with meta.
No, as a standalone "label" on line 1 or line 2.Also no for the layout. If user has two or more labels, profile card does not show all. Tries to center the text and it becomes unreadable.
Is it possible to cutomize this?

Have you tried enabling the Labels for a View Person form also? This should make it available on the Info Line builder.
