Database query error

I get a “Database query error” then I go to “on site” , “ email” or “ push” in
Studio->Notifications
we are on 12 B1

Im getting data base errors as well in different areas of site.
Specifically as of now in sounds

Did you try adding new fields to your forms??

No haven't really looked in to. Its a default post update thing. Database errors as well. I see the underscore/titles thing mostly in studio, especially in the new side menu feature.
