The admin may define the groups for certain users via the Persons -> Manage -> Manage All -> Manage Audience area.

Here is the screenshot. I signed up as a normal member using facebook connect and I can only see the "G" option. I, as the admin, also deleted the "R" and "X" options from the fields. I, as a normal user, should still be able to see "PG" and "PG-13".

It depends on the value of the Date of Birth field. Do you use it during the profile creation?
