Yes, it needs SMTP or preferably a transactional email provider (sending from your own server will quickly result in all email routing to spam folders). Giving massmailer to all users is technically possible but I can’t imagine how it can be feasible. It allows emailing everyone on the site, so it’s a feature you want to restrict to admins only.

  Massmailer is atm an admin module, not a public one. As Andrey mentioded above
  Going with UNA frame so SMTP is always the best solution to have control of your own mails.
  MASS MAILER is not Mail Chimp. Right now mass mailer is best for admins and mods and that is perfect to avoid any conflicts.
  To Mass Mail to your users of your UNA frame/site there are many other ways.... like mail chimp
