Situation: User A has permission to a secret group. User B does not have permission to a secret group. User B is friends with User A.
Is there a reason why User B will get a notification that User A voted on a discussion in a secret group? It seems like if the group is secret, no one without permissions should get notifications about activity in the group--even if they are friends with someone who does.

We had an issue something akin to this some time ago, but with the Reports section. When a moderator commented on a report, the person who made the report could see the notification alongside the moderators friends. Haven't figured out why.

I couldn't reproduce this part on my side. Only the group's creator has been notified about a new like from another user (user A). User A has a friend user B and user B didn't get any notifications about the likes of user A in the secret group.
